Baptism Request

Request baptism for a child under the age of 7

Thank you for your desire to have your child baptized at St. Albert the Great Catholic Church.

As a community we welcome new members of our faith with great joy. In order to best accommodate the baptism of your child we ask you to complete the form below as accurately and completely as possible.

Baptism is a Sacrament which brings a new life into the body of Christ, the church, and as such is appropriately a communal event. Therefore, although a baptism may be performed at any time, the preferred time to conduct a baptism in the Catholic Church is as part of the communal celebration of the Mass.

There is no charge for any baptism performed during the celebration of the Mass. However, a $50 donation is suggested for the clergy performing the baptism.

Baptisms outside of Mass require a fee to cover the cost of using the church facilities and a stipend for the clergy performing the baptism. The fee schedule is as follows and must be paid before a baptism will be scheduled:

  • Registered parishioners: $100.00
  • Non-parishioners: $150.00

Parents and Godparents

Parents and godparents who have not attended a baptism class must do so before a baptism will be scheduled.

Please note: there is a $10 fee for class materials, payable at signin.

If either the parents or godparents are from another parish or reside outside the Diocese of Reno, you must provide a certificate or letter from your parish, signed by your pastor, attesting to your attendance at a Baptismal Preparation class.

Please ensure that all fees (if required) have been paid before the Baptismal Preparation class.

PARENTS: For the Baptismal Preparation Class, please bring a COPY of the child's Birth Certificate.

Godparent's Information
In the baptism of children, godparents are to be present in order to represent both the expanded spiritual family of the one to be baptized and the role of the Church as a mother. As occasion offers, godparents help the parents so that children will come to profess the faith and live up to it. Godparents, together with the parents, are called to profess the faith of the Catholic Church, in which the child is to be baptized.
Godparents are persons, other than the parents, designated by the parents or whoever stands in place of parents, or, in the absence of these, by the pastor or the minister of baptism. Only one male or one female or one of each sex may be employed as a godparent.
Those so designated as godparents must have the capability and intention of carrying out the responsibility of a godparent and be mature enough to do so. A person chosen to be a godparent must be Catholic, sixteen years of age or older, must have received the three sacraments of initiation (Baptism, Eucharist, and Confirmation.) and be living a life consistent with faith and with the responsibility of a godparent.
At the request of the parents, a baptized and believing Christian witness, not belonging to the Catholic Church, may act as a Christian witness along with a Catholic godparent. (Code of Canon Law: Canon 872, 873, 874)
